Kota Connections is a family-run, dually enrolled (245D & VRS-LUV) provider in Lakeville, Minnesota. We help adults with disabilities work, live independently, and stay connected to their community.

Most providers assign one staff per client. We do the opposite. A team of four to six rotates, so a staffing change never leaves anyone without support, and we always have the people to run a full calendar.
We build employment services around what each person actually wants to do for work. In 2020, APSE recognized our approach with its Organizational Best Practice Award, citing a 100% job placement rate.
All delivered under our active Minnesota DHS 245D license. Funding through VRS-LUV, CDCS, Dakota County, State Waiver, Brain Injury (TBI), CADI, CAC, and DD waivers is commonly accepted.
Exploration, development, and on-the-job support to help individuals find and keep competitive employment in the community.
Scheduled visits one to three times per week for clients who live on their own lease, with rotating staff so coverage never drops.
Transportation, in-appointment support, documentation, and follow-up tracking for clients managing ongoing medical care.
Group outings, KC meetings, and day support, built from what clients tell us they want to do. Transportation and group staffing included.
