Privacy Policy

Effective date: August 12, 2026
Last updated: August 12, 2026

Kota Connections, Inc. (“Kota Connections,” “we,” “us,” or “our”) operates this website. We are a family-run, dually enrolled disability services provider in Lakeville, Minnesota, serving adults with disabilities across Dakota County.

This policy explains what we collect on this website, why, and what you can ask us to do about it. It is written to be read, not skimmed past.

The short version

  • This website has one contact form. It asks for your name, email, phone, and a message. Nothing else.

  • We do not sell your information. We do not share it with advertisers. We do not run advertising or analytics trackers on this site.

  • Please do not send diagnoses, medical history, or other private health details through the contact form or by regular email.

  • Client service records are covered by a separate notice, not by this page.

  • Questions? Call 952·469·1555 and a person will answer.

What this policy covers, and what it does not

This policy applies to this website only — kotaconnections.net and the information you submit through it.

It does not cover the records we keep as a licensed service provider. Information about the people we support — support plans, case notes, medical and behavioral information, county and waiver documentation, billing records — is protected health information handled under separate federal and Minnesota law, including the Health Insurance Portability and Accountability Act (HIPAA) and the Minnesota Health Records Act.

Those records are governed by our Notice of Privacy Practices, which is a different document. If you are a client, a guardian, or a case manager and you want a copy, call 952·469·1555 or email referrals@kotaconnections.net and we will send one.

Information you give us

Through the contact form. Our contact page has a single form. It collects:

  • Your name

  • Your email address

  • Your phone number

  • Whatever you write in the message field

That submission is emailed to our staff and stored on the website’s hosting account. We use it for one purpose: to reply to you and, if you are asking about services, to begin a conversation about them.

By phone, fax, email, or mail. If you call, fax, email, or write to us, we keep what you send us so we can respond and, where relevant, keep a record of the request. Our fax line is 952·469·1478 and our mailing address is 8500 210th St W, Suite 148, Lakeville, MN 55044.

Referral packets. Referral forms on this site are documents you download, fill out, and return to us directly. They are not submitted through the website, and the website never sees their contents. Return them by fax, by mail, or by arrangement with our office — see the note on sensitive information below.

Employment inquiries. If you contact us about working here, we keep the application materials and correspondence you send. We use them to evaluate candidates and, if you are hired, to start employment records.

A note on sensitive information

Regular email and web forms are convenient but not secure. Please do not use the contact form or ordinary email to send diagnoses, medical or behavioral history, medication lists, guardianship or court documents, Social Security numbers, insurance or Medical Assistance numbers, or details about a person’s disability.

If you need to send something like that, call us at 952·469·1555 first and we will tell you the right way to get it to us. It usually takes one phone call.

If sensitive information does reach us through the website, we treat it with the same protections we apply to our service records and we do not leave it sitting in the website’s database.

Information collected automatically

Server logs. Our web host records standard technical information when a page is requested: IP address, browser and device type, referring page, and the date and time. This is ordinary hosting activity used for security, spam prevention, and diagnosing problems. We do not use it to build a profile of you.

Cookies and similar technology. This website uses only what it needs to function. WordPress and our page builder may set cookies to keep the site working correctly, and our accessibility tool may store your display preferences — such as larger text or higher contrast — so your choices persist while you browse.

We do not currently run advertising pixels, remarketing tags, or third-party analytics on this site. If that changes, we will update this page and, where required, ask for your consent first.

You can block or delete cookies in your browser settings. Core pages of this site will still work if you do, though accessibility preferences may not be remembered.

Do Not Track. Because we do not track visitors across other websites, there is nothing for a Do Not Track signal to switch off.

How we use information

We use what we collect to:

  • Reply to your question, call, or message

  • Respond to referrals and begin the intake process

  • Coordinate with case managers, counties, and funding sources when that is part of arranging services

  • Review employment inquiries

  • Keep the website secure and working

  • Meet our recordkeeping, licensing, and reporting obligations as a Minnesota DHS 245D provider

We do not use website information for advertising, and we do not use it to make automated decisions about anyone.

How we share information

We do not sell personal information. We never have.

We share information only in these situations:

  • With your permission, or with the permission of a legal guardian or authorized representative

  • With counties, case managers, and funding sources as needed to arrange, deliver, and bill for services, consistent with the applicable authorizations and releases

  • With service providers who operate parts of our business, such as our web host and email provider, and only as needed to do that work

  • When the law requires it, including mandated reporting of suspected maltreatment of a vulnerable adult under Minnesota law, responses to lawful requests from licensing or regulatory agencies, court orders, and subpoenas

  • To protect safety, when there is an urgent risk to the health or safety of a client, a staff member, or the public

How long we keep information

Contact form submissions and general inquiries are kept only as long as we need them to respond and to maintain a reasonable record of the request, then deleted.

Client service records, employment records, and billing records are kept for the periods required by Minnesota DHS licensing rules, HIPAA, and other applicable law. Those periods are longer, and they are set by regulation rather than by us.

How we protect information

Our website is served over an encrypted HTTPS connection. Access to the website’s administrative tools and to our email is limited to staff who need it.

We are a small organization, and that is part of the answer here: fewer people handle your information, and the people who do are long-tenured staff who know the families they work with. No website can promise perfect security, but we take reasonable steps and we do not collect information we do not need.

Guardians, representatives, and vulnerable adults

Many of the people we support have a legal guardian, conservator, or authorized representative. Where someone holds that authority, they may exercise privacy rights on that person’s behalf, and we will ask for documentation confirming it.

We also support the right of the people we serve to have a say in their own information. Where a person can make or participate in decisions about their own records, we involve them.

Children’s privacy

Our services are for adults. This website is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has sent us information through this site, call 952·469·1555 and we will delete it.

Your choices and your rights

You can:

  • Ask what we have. Contact us and we will tell you what website information we hold about you.

  • Ask us to correct it. If something is wrong, tell us and we will fix it.

  • Ask us to delete it. We will delete website inquiries on request, unless we are required to keep them.

  • Stop hearing from us. Ask and we will stop contacting you, except where we must respond to something you initiated.

For client service records, access, amendment, and accounting-of-disclosure rights are described in our Notice of Privacy Practices and are governed by HIPAA and the Minnesota Health Records Act.

To exercise any of this, call 952·469·1555 or email referrals@kotaconnections.net. You do not need to submit a formal written request to get a real answer from us.

Accessibility

This site includes accessibility tools, and we want it to work for the people we serve. If any part of this website — or this policy — is difficult to use or understand, call 952·469·1555 and we will help you directly and work on fixing it.

This website links to county pages, state resources, and other outside organizations. We do not control those sites and this policy does not apply to them. Please review their privacy policies.

Changes to this policy

If we change this policy, we will post the revised version here and update the effective date at the top. Significant changes affecting how we handle information about the people we support will also be communicated directly to clients and their representatives.
