The current referral form is in the document resource library. Submit it along with a current Community Support Plan or equivalent.
Send to referrals@kotaconnections.net. We confirm receipt the same business day.
Once reviewed, we confirm whether we can meet the needs in the referral and schedule an intake meeting with the client, guardian, and team. Typical referral-to-start: 1–3 months, can be expedited.
We treat case managers as part of the team from intake forward.
Records or a contact with a timeline provided within one business day of request.
Every support, search, or planning visit emailed to the team. Other services are updated as needed.
No phone tree. No separate intake routing. The same family that runs Kota answers the phone.
We tell you when we can’t meet a need, and where we can, we’ll point your team toward another Dakota County provider who might be a better match.
