You know the person better than anyone. This page is for you, the parent, the spouse, the sibling, the guardian, the family friend who has been there from the start. Here is how Kota fits into the support circle you have already built.
Caregivers often ask a version of the same question: where does the provider's role end and mine begin? Here's how the line falls at Kota.
Physical personal care (Kota staff do not provide hands-on PCA)
Anything not in the active service plan signed at intake
Whether the client wants you copied on visit notes, or kept private
How decisions get made when the team disagrees on a path
Communication is what families ask about most. The default at Kota is that the client decides what is shared with their family or guardian and what stays private. Adults receiving services have the right to decide who hears about their day, and we honor that. Most clients are happy to keep their family in the loop, especially when something changes.
Visit notes when there are issues or concerns to relay.
Every support, search, or planning visit, emailed to the team.
The right person to contact depends on what you need. Here is the short version.
Call the office at 952·469·1555. The same family runs Kota and answers the phone, your call routes quickly.
Loop in the case manager first. Kota can adjust within the existing plan, but plan-level changes go through the county.
Call us directly. After hours, the line routes to on-call. Always leave a message, they get sent to staff by email. Emergency? Call 9-1-1.
Email kota@kotaconnections.net or call the office. Usually turned around within a business day.
Caregiving for an adult with a disability is its own kind of work, and the people who do it well usually have help. We don't run a caregiver support group, but we're happy to point you to what's available in Dakota County and statewide.
