STARTING A REFERRAL

Case managers and social workers complete the referral packet and email it to referrals@kotaconnections.net. Family members not sure where to start, call us and we will walk you through it.

2. Have the case manager complete it

3. Email the completed packet to referrals@kotaconnections.net

4. We follow up to schedule an intake meeting
