About us
 
Kota Connections, Inc. has been providing services for individuals with disabilities since 1992.  We are located in the heart of downtown Lakeville, MN and assist clients living in Dakota, Hennepin, and Scott counties.  We currently provide SILS, In-home, and Waivered Services.  Whether living independently, with family or friends, or in a foster care environment, we provide tailored services to enable our clients to achieve their goals and live successfully within their community.

Kota Connections exists to improve the lives and independence of the special needs adults we serve. We maintain a consumer-first focus and are committed to providing individualized services to enable our consumers to reach their goals and improve their independence.

 

 

Services
  • Apartment Checks
    •  
      We assist in improving independent living skills in the areas of personal care, meal planning, shopping, cooking, money management, clean and organized living, and community involvement.

 

    • We tailor our services to meet the unique needs of each of our clients and enjoy finding one-of-a-kind solutions to help our clients succeed.

 

    • We don't do it for you!  We respect our clients as independent adults.  As such, our philosophy is to employ methods that assist our clients in their daily living not to complete daily tasks for them.  Our aim is to help our clients learn the necessary skills for successful independent living. 

 

  • Medical Appointments
    • We ensure that our clients receive timely routine as needed medical and dental care.

 

    • We provide transportation to and from appointments and our direct support professionals can provide support and/or communication assistance at client appointments as needed.

 

    • As necessary, we coordinate and schedule all routine and follow up appointments on behalf of our clients.

 

  • Social Activities
    • We all need to let loose every once in a while!  At Kota Connections we believe that enjoyable social outings are an essential part of a balanced and happy life.  As a result, we coordinate multiple activities each week for our clients, many at little or no cost.

 

    • We research, coordinate, and schedule activities on a quarterly basis.  Clients recieve activity calendars outlining our upcoming activity offerings.

 

    • We provide transportation to and from each planned activity, as needed.  Our direct support professionals also provide client support at each outing as needed by each client. 

 

    • We enjoy watching our clients build friendships.  We structure our activities to encourage our clients to interact with each other and our staff to facilitate and support as needed. 

 

  • Softball League
    • Each year from April - August we coordinate and run a softball league for special needs adults.

 

    • The league consists of an 'A' level and a 'B' level.  Both levels are made up of teams from Kota Connections and other providers.
